Through personalised funding and services tailored to individual needs, the program provides access to practical support that helps people maintain their independence, wellbeing, and connection to their community.

Practical help with personal care, household tasks and daily routines so you can stay comfortable, independent and supported at home. This includes assistance with cleaning, getting to appointments, social support, grooming, meals, mobility, errands and general wellbeing.
Professional nursing and clinical support delivered at home to manage medications, wounds, chronic conditions and ongoing health monitoring.
Equipment and home adjustments — such as rails, ramps, mobility aids and adaptive technology — that improve safety, comfort and accessibility around your home.

The Support at Home Program is supported by Australia’s new Rights-Based Aged Care Act (2024), which legally protects your rights when receiving Government-funded aged care.
