Restorative Care Pathway

Rebuilding Strength, Independence and Confidence

Helping You Recover and Stay Independent for Longer

Ageing does not mean losing independence. Many health challenges can be improved through targeted support, rehabilitation, and practical interventions.

The Support at Home Program includes a dedicated Restorative Care Pathway aimed at helping older Australians regain function, improve wellbeing, and reduce the need for long-term support.

What is Restorative Care?

Restorative Care focuses on improving a person’s ability to carry out everyday activities and maintain independence.

Rather than simply providing ongoing assistance, restorative care helps people rebuild skills, strength, confidence, and capacity.

The pathway uses a wellness and reablement approach that encourages people to remain as active and independent as possible.
