Restorative Care Pathway

Rebuilding Strength, Independence and Confidence

Helping You Recover and Stay Independent for Longer

Ageing does not mean losing independence. Many health challenges can be improved through targeted support, rehabilitation, and practical interventions.

The Support at Home Program includes a dedicated Restorative Care Pathway aimed at helping older Australians regain function, improve wellbeing, and reduce the need for long-term support.

Palliative & End-of-Life Care

Palliative & End-of-Life Care

Compassionate Support During Life’s Most Important Moments

For many Australians, home is where they feel most comfortable, secure, and connected to loved ones. When facing a life-limiting illness, remaining at home can provide comfort, familiarity, and the opportunity to spend meaningful time with family and friends.

Carexcell’s Palliative and End-of-Life Care services support individuals and families through this important stage of life with compassion, dignity, and respect.

The Support at Home Program includes a dedicated End-of-Life pathway to help participants access increased support and services when they need them most.

Home Modifications & Assistive Technology

Making Your Home Safer, More Accessible and More Independent

Supporting Independence Through the Right Equipment and Home Adaptations

As we age, changes in mobility, strength, balance, vision, or health can make everyday activities more difficult. The right assistive technology and home modifications can make a significant difference, helping older Australians remain independent and safe at home.

The Support at Home Program includes a dedicated Assistive Technology and Home Modifications pathway designed to improve safety, accessibility, and quality of life.

Clinical Care & Nursing

Clinical Care & Nursing

Expert Nursing Care and Clinical Support in the Comfort of Your Home

Professional Healthcare Delivered Where You Feel Most Comfortable

As health needs become more complex with age, many older Australians require ongoing clinical support to remain safely at home. Carexcell’s Clinical Care & Nursing services provide professional healthcare delivered by qualified nurses and allied health professionals, helping you maintain your health, independence, and quality of life.

Under the Support at Home Program, Clinical Supports are fully government funded for eligible participants, meaning there are no out-of-pocket contributions for approved clinical services.

Private and Brokerage

Private Services

Private home care services are the same quality and calibre of service that people receive from government funded packages, they are simply paid for directly by the client, rather than the government.

People access private home care services for a variety of reasons. Maybe their current government funded services cannot provide as many hours as wanted. Maybe they are waiting on a package being allocated. Sometimes people accessing private home care are not eligible for government or insurance packages. Whatever the reason, Carexcell’s private home care services are of the highest quality and support people to remain independent at home.

Coordination of Supports

Carexcell’s Coordination of Supports service is more than just connecting you with supports and providers—it’s about empowering participants to take control of their NDIS journey, build independence, and achieve their goals.

Palliative Care

Palliative Care

Palliative care is a specialised form of medical care for people with an active, progressive and advancing disease that is life limiting. It’s not just any care; it’s about focusing on providing relief from symptoms and improving the quality of life for both the person and their family. This kind of care is for anyone living with a serious illness, like cancer or heart failure, and it’s not only about physical relief but also emotional and psychological support.
